Please fill out the Family Practice Scholarship Application fully and completely.

As a reminder the Family Practice Scholarship is available only to students who are:

  • West Virginia residents as defined by their legal state of residence upon acceptance to WVSOM;
  • Entering year two of their medical education at WVSOM;
  • Intends to practice family medicine;
  • In good academic standing and in the upper two thirds of his/her class academically.
